We congratulate the U.S. Preventative Task Force on its recent recommendation
in regards to prostate cancer screening in young healthy males. We recognize the
Herculean task put before the members of the task force, and applaud them for
bringing this controversial topic to the forefront of public discussion.
However we are concerned that their recommendations to advise against
routine screening may be misguided and misinterpreted. Their final conclusion
was based on reviewing previous scientific trials - that even they admitted were
fraught with contamination and potential biases. In their concluding sentences,
they even mention that these studies had not reached maturity and that over the
next decade, as these studies mature, they will give a clearer picture as to the
potential benefit of prostate cancer screening.
We are concerned that lost in this report is that approximately 32,000
American men will die of prostate cancer this year, and that prostate cancer is
the second leading cause of cancer death in American men. We are also concerned
that this report did not sufficiently take into account what benefit prostate
cancer screening and early detection may have on decreasing or preventing the
debilitating effects of advanced or metastatic prostate cancer. In countries
where little prostate cancer screening is done, half of the men diagnosed with
prostate cancer die from their cancer. Since the introduction of Prostatic
Specific Antigen (P.S.A.), a blood test used for prostate cancer screening, the
incidence of death from prostate cancer has declined with no other clear cause,
and the task force trial did not address this fact.
We do agree with the task force that not all men need to be screened for
prostate cancer, and that certain men with prostate cancer should be observed
and not treated - initially or perhaps ever. We also agree with their unstated
belief that we as a medical community, as a whole, need to continue to research
and refine our diagnosis and treatments to improve the health outcomes of our
patients and minimize any potential side effects to our treatments.
Eliminating prostate cancer screening will not eliminate disease. The
urologist at Atlantic Urological Associates feel strongly that if you are
concerned or at risk for prostate cancer, you should consult your urologist to
discuss whether prostate cancer screening is in your best interest.
